Mail Forwarding Request Form
If you are a former guest or community member that is currently receiving mail at either the Craig’s Doors Resource Center (434 N Pleasant St) or the University Motor Lodge (345 N Pleasant St) please fill out the form below and we will forward your mail to the address of your choosing.
Please note that this service is a one-time courtesy for regular mail envelopes only (not packages) - you will need to complete an official change of address with the United States Postal Service (click here) to ensure your mail is automatically forwarded in the future.
If you do not have a forwarding address or would prefer to pick your mail up in person, please give us a call at 413-256-0704 ext. 8 to arrange a time to stop by the Resource Center. All unclaimed mail not associated with a forwarding address will be held for two weeks before being returned to sender.
